New Patients


 

Thank you for choosing Indiana Neurology and Pain for your medical care. In an effort to help our patients understand our office policies, we have prepared information for you below. If you have questions about anything, we encourage you to call our office and we’ll be happy help you.

Registration

Patients will need to register with our office for your initial visit with Dr. Kundi. This can be done in one of two ways: 1) You may register through our secure patient portal online or 2) you may download our patient registration forms, complete them and then bring them in to the office with you for your initial visit.

When you visit us for the first time, please be sure to bring the following items:

  • A complete listing of each of any medications your are currently taking, including any over the counter medications.
  • Your insurance card if you have medical insurance.
  • Your completed patient registration paperwork ( if you did not use the patient portal to register).

Appointments

We see patients by appointment. Call our office to schedule an appointment any time during regular office hours. If you find that you are unable to keep your scheduled appointment, please contact us within 48 hours of your appointment so that we may offer it to someone else in need.

Financial/Insurance

We accept most major health insurance plans. If we are participating with your health insurance plan, we will file your insurance for you. Please be prepared to pay any co-payment, deductible or non-covered services on the day of your appointment. For your convenience, we gladly accept cash, checks, debit and credit cards.

Prescription Refills

We ask that you allow at least 48 hours for prescription refills. This gives Dr. Kundi time to review your medical chart to determine if a follow-up appointment is needed or if a refill can be called in to the pharmacy. When calling, please provide us with the following information:

  • Name of your Pharmacy
  • Pharmacy phone number
  • Name and dosage of your medication

Please understand that, as a general rule, we do not phone in prescriptions after hours or on weekends.
